At a glance
- Blank, editable CSV
- Print or save as PDF
- No account required
Choose a format and keep one event per row
The template starts empty and does not send your completed spreadsheet or printed notes to GlowBack. Your spreadsheet app, browser, storage service, or recipient may handle files you choose to save or share.
Download the CSV and open it in a spreadsheet, or use Print or save as PDF for the blank table below. On a device without that button, use your browser’s print or share menu.
Use one row per dated event. Add the medication and amount only when recording a dose you actually took. Leave unrelated fields blank.
For weight or symptom-only entries, fill the date and the relevant field. Write units and your own severity scale consistently.
Save the file in a location you control. Review the notes and personal details before sharing it.

Examples of what belongs in a row
These are illustrative record types, not real user records or a treatment plan. If you are unsure what happened, describe the uncertainty in notes instead of entering an invented dose or measurement.
| Event | What you could record | What to leave open |
|---|---|---|
| A dose you actually took | Actual date/time, medication, recorded amount/unit, and site. | Use your own prescribed instructions; the sheet provides no dose recommendations. |
| A symptom observation | Date/time, symptom, your severity score, and context. | A note does not establish the cause. |
| A weight measurement | Measurement date, value, and kg or lb. | Do not infer a missing measurement from nearby values. |
When a sheet is enough, and when an app helps
A sheet works well for a simple list you maintain yourself. GlowBack can keep dose, symptom, food, water, weight, and progress-photo records together, with reminders and other features depending on platform and plan.
This downloadable sheet is separate from the app’s PDF/CSV export. It is not an import file for GlowBack, does not automatically synchronize, and does not calculate medication levels.
